Treasurer for 2016-2017 School Year is:

This form is to be completed for reimbursement of
expenses incurred on behalf of the PTA and must be
accompanied by any/all receipts.

For use by Committee Chairs, this form is to be
completed as the proceeds from an event are
counted and is to be submitted to the Treasurer
along with the monies collected.
